How to refer
- Send a referral through our secure online form — it takes just a couple of minutes.
- For Medicare rebates, include a GP Chronic Disease Management (CDM) plan and referral.
- We confirm receipt within 24–48 hours and arrange an initial assessment.
We close the loop. You'll receive a concise, evidence-based assessment summary and ongoing progress reports, with collaborative goal-setting wherever it helps the child.
